PRESENTATION OF VALID IDENTIFICATION CARDS
National Administrative Register June 25, 2004
For purposes of security and uniformity, you are hereby directed to require at least two (2) valid identification cards hereunder listed before issuing a duplicate Official Receipt (OR) and/or Certificate of Registration (CR):
1. Valid Driver’s License;
2. Voter’s ID Card;
3. Postal ID Card;
4. PRC ID;
5. Passport;
6. Credit-card type Government ID bearing both the picture and the signature of the holder/owner;
7. Senior Citizen ID;
8. School IDs accompanied by current registration card;
9. Company ID bearing the both the picture and the signature of the holder; and
10. SSS ID.
All orders and/or memoranda that are in conflict herewith are hereby superseded.
This Memorandum Circular takes effect immediately.
