INTING, S. B., J.:[*]

Before this Court is a Petition for Review[1] filed on April 30, 2014. However, the Petition suffers from the following infirmities:

1) The Docket Fees paid is short of the amount of Php1,150.00 as payment for the prayer for injunctive relief;

2) Both the Verification and Certification of Non-Forum Shopping
are defective because the Notary Public before whom the aforesaid
documents were subscribed and sworn to failed to indicate the province
or city where he was commissioned pursuant to section 2 (c ) of Rule
VIII of the 2004 Rules on Notarial Practice;

3) The certified true copy of the Complaint for Disability
Benefits, etc. filed by the herein Respondent against the Petitioner
before the National Conciliation Mediation Board which would support the
allegations in the Petition was not attached thereto.

IN VIEW OF THE FOREGOING, for being formally defective the Petition for Review is hereby DISMISSED pursuant to Section 7, Rule 43 of the Rules of Court and the case is deemed CLOSED and TERMINATED.

SO ORDERED.
