Before this Court is a Petition for Review^[1] filed on April 21, 2014. However, the Petition suffers from the following infirmities:

1) Both the Verification and Certification of Non-Forum Shopping and the Affidavit of Service are defective because the Notary Public before whom the aforesaid documents were subscribed and sworn to failed to indicate the province or city where he was commissioned pursuant to section 2 (c ) of Rule VIII of the 2004 Rules on Notarial Practice;

2) Only the original duplicate copy of the Decision dated December 26, 2013 of the Regional Trial Court of Manila, Branch 27 is attached as Annex “A”; the certified true copy or original duplicate copy of the Decision dated May 23, 2013 rendered by the Metropolitan Trial Court of Manila, Branch 21 is not attached;

3) Only the xeroxed copy of the assailed Resolution of the Civil Service Commission was attached to the Petition;

4) The entire legible duplicate copy of the records of appealed Civil Case No. 13-130467 filed before the Regional Trial Court , Branch 27, Manila, inclusive of the oral and documentary evidence which would support the allegations in the Petition was not attached thereto.

IN VIEW OF THE FOREGOING, for being formally defective the Petition for Review is hereby DISMISSED pursuant to Section 3, Rule 42 of the Rules of Court and the case is deemed CLOSED and TERMINATED.
